Cleft lip and palate represent complex congenital craniofacial anomalies with significant implications for speech production, auditory function, feeding, craniofacial growth, and psychosocial development. Although reconstructive surgery remains fundamental to anatomical correction, optimal long-term outcomes require coordinated multidisciplinary management extending throughout childhood and adolescence. This narrative review aimed to critically examine the scientific evidence regarding staged surgical reconstruction, velopharyngeal competence, and comprehensive developmental follow-up in pediatric patients with cleft lip and palate, incorporating international research and contributions from Latin America. A descriptive, analytical, and comparative approach was adopted, integrating 20 scientific publications addressing surgical timing, reconstructive techniques, postoperative velopharyngeal insufficiency, auditory outcomes, nutritional development, and quality of life. The findings identified an association between earlier primary palatoplasty and reduced velopharyngeal insufficiency, with a multicenter randomized trial reporting postoperative insufficiency rates of 8.9% following repair at six months and 15.0% following repair at twelve months. Comparative evidence also demonstrated differences in functional failure rates between Furlow double-opposing Z-plasty and straight-line intravelar veloplasty, although methodological heterogeneity limited definitive conclusions regarding surgical superiority. Persistent velopharyngeal dysfunction remained a clinically relevant complication requiring individualized assessment and, in selected cases, secondary reconstruction. Longitudinal evidence additionally demonstrated changes in auditory thresholds across childhood, while nutritional and growth outcomes varied according to anatomical characteristics and clinical circumstances. Latin American investigations emphasized the relevance of sociodemographic conditions and access to specialized services. Overall, the findings support an individualized, evidence-informed model of care integrating anatomical reconstruction, functional rehabilitation, and continuous developmental surveillance. Further prospective multicenter investigations using standardized outcome measures are required to strengthen clinical recommendations and improve long-term functional outcomes and quality of life.
